In Eurekos, questionnaires do more than collect answers—they can actively support processes and facilitation across the platform. No matter where a questionnaire is used (onboarding, training requests, applications, in-course activities, or feedback), you can apply built-in rules to turn responses into structured follow-up actions.
This means questionnaires can help route and qualify requests, support review and approval steps, and drive automation such as enrollment/auto-enrollment, segmentation and recommendations—so the right people get the right next step with less manual work.
Profile Enrichment At Login
Use questionnaires to collect or validate information when users log in. This is ideal when you need details that may not exist in your CRM, HR system, or provisioning setup—such as role-specific context, preferences, internal classifications, acknowledgements, or additional consent.
Typical outcomes:

- Better targeting of learning content
- Improved reporting and compliance documentation
To set up:
Onboarding rules are configured under Settings → Onboarding, where you define which audience should receive a questionnaire and when it should be shown. Responses can then be used to influence recommendations, the Storefront experience, and training assignments.
Training Request Intake (On-demand Requirements)
Create an intake form to capture on-demand training requests—especially for group or customer training. A well-designed questionnaire reduces back-and-forth and helps you respond faster and more professionally.
This can range from a simple, gated request form that supports manual follow-up and assignment, to a structured intake for custom group programs that require planning in advance (e.g., scope, audience size, format, and scheduling).
Common questions include:
- Delivery format (virtual, on-site, blended)
- Location (if relevant)
- Number of participants
- Product(s) or topic(s)
- Desired schedule/timeframe
- Goals, current challenges, and other open-ended prerequisites
To set up:
On-demand scenarios are configured per training activity in the Activity features section. Here, you select which questionnaire should act as the intake form—or you can use the platform’s standard Contact Form (Settings → Forms → Contact) to collect requests.
The request intake replaces the enrollment option on the activity description page. Depending on the setup, the system sends a standard request email to the relevant administrator(s).
Application and Approval Workflows
Questionnaires can support structured application flows for programs, academies, certifications, or limited-seat training. This creates a scalable way to review submissions consistently and make decisions based on comparable data.
This may include:
- Collecting applicant details and background information
- Uploading supporting documentation
- Capturing acceptance of privacy policies, terms, and conditions
- Pre-marking candidates for review and shortlisting
To set up:
As a prerequisite, Self enrollment must be enabled for the training activity under Activity features. This unlocks the option Participation approval, which lets you require an applicant to complete a questionnaire as part of the application.
- When a questionnaire is required, you can use it as an intake form and enable the advanced application approval workflow. This supports structured, manual review and approval of applicants and is described in detail in separate articles. It’s a comprehensive feature set intended for more complex application processes.
- Simpler approval (no questionnaire): If you enable Participation approval without requiring a questionnaire, you get a lighter approval flow—typically routed to a Manager role or a specified email recipient. This works well when you don’t need additional information, but still want a formal sign-off process (for example due diligence related to relevance, cost, customer training procedures, or compliance requirements).
Training Feedback and Evaluation (Instructor + Participants)
Collect feedback as part of a workflow after training delivery—either between sessions/modules or as a final evaluation at the end of the program.
Examples of what to measure:
- Instructor performance (self-evaluation vs evaluation by participants)
- Relevance and quality of materials
- Session structure and pacing
- Learning outcomes and confidence levels
- Suggestions for improvement
To set up: Training Feedback is an Activity- or Module-level feature. This means you can configure it at the learning-path (program) level to request completion of a questionnaire when the entire program is completed.
- If the activity contains multiple modules (e.g., calendar events, virtual events, or course modules), you can add a Questionnaire as a feedback step and configure settings such as when it appears, and whether completion is required to receive a certificate, along with other related options. You can also present a separate questionnaire for instructors to capture their feedback independently of learners.
- If the training activity contains only one module, the feedback questionnaire is configured on the module itself (use the [Gear] icon to access module features). This also makes it possible to collect pulse feedback on specific parts of a program when needed.
In-course Data Collection (Assignments and Facilitation)
Questionnaires can be used during the learning experience—not only before or after it. This supports blended and virtual instruction where instructors need structured input throughout delivery.
Examples:
- Participant reflections (individual assignments)
- Group exercises, sharing and discussing feedback
- Facilitator-driven check-ins during modules
- Data capture for instructors to guide the next session
To set up:
Use the [Button] tool to insert a button on a page as a separate object, or add manual links on course pages to share a questionnaire. If you create manual links, use an anonymous questionnaire, since the activity relationship is not technically preserved in this way.
Surveys For External or Anonymous Audiences
Questionnaires can be shared anonymously or distributed via email links. This is useful when you want input from people who may not be registered in the LMS or when anonymity improves response quality.
Typical scenarios:
- Event or webinar follow-ups
- Prospect/customer feedback loops
- Partner surveys
- Internal pulse checks where anonymity matters
To set up:
From Course Administration → Questionnaires, open [More] next to the questionnaire and select [Copy URL]. The copied link can be shared with anonymous respondents, so it does not require login or even a registered user.
Since you can’t rely on profile data for anonymous users, make sure the questionnaire asks for any key details you need (e.g., name, email, organization, context).
Additional User Profile Intake (Use a Questionnaire for additional info)
Eurekos supports using questionnaires to collect additional profile information for organization-specific needs—beyond the platform’s standard master data such as name, email, and default profile fields and vocabularies. If you need to capture more detailed information (for example experience, interests, preferences, or evaluation-related inputs), questionnaires provide a flexible and scalable way to do so.
Users can review and update this information at any time by editing their profile. When profile data is updated through a questionnaire, the same questionnaire capabilities apply—meaning validation, rules, and automation can still be triggered based on the submitted answers.
To set up: Go to Settings → General and enable the option [Supplemental questionnaire] to activate the feature. Then select the questionnaire you want to use and click [Save]. The intake form is now available when a user edit his profile.
The same questionnaire can also be reused in other processes whenever you need to collect or update this information.
