Training Feedback lets you ask participants or instructors structured questions about a Course, Event, or complete learning journey. You choose the questionnaire, when it becomes available, whether it must be completed before a certificate is issued, and who should be reminded or alerted. What Training Feedback is
Training Feedback connects a questionnaire to a delivered Training Activity or one of its supported modules. It gives registered participants a controlled way to evaluate the learning experience and keeps the response connected to the training context in analytics and reports.
Typical feedback topics include:
- Relevance to the participant's role
- Quality and clarity of the learning material
- Instructor or facilitator effectiveness
- Pace, structure, and duration
- Location, equipment, and virtual delivery
- Confidence before and after training
- Whether the learning objectives were achieved
- Suggestions for improvement
- Likelihood to recommend the training
Training Feedback can support both self-paced and instructor-led learning. It can measure one Course or Event module, several points across a learning path, or the participant's overall view of the completed program.
Training Feedback is a delivery and quality workflow
Attaching a questionnaire is only the collection step. Decide who will review the results, how often, which results require action, and how improvements will be documented. Without that operating process, a high response rate produces data but not necessarily better training.
Choose the correct feedback tool
Several Eurekos features collect opinions or responses, but they are not interchangeable.

You can combine these features deliberately. For example, enable Rating for a quick public satisfaction signal and use Training Feedback for detailed internal evaluation. Do not ask the same question through both unless you need two distinct measures.
Decide where feedback belongs
The Questionnaires feature can be configured at:
- Course module level
- Event module level
- Training Activity level for a multi-module learning path
The selected level determines what the participant is evaluating, when the feedback becomes available, which schedule and completion signals apply, which certificate can be held, and which activity or module context appears in reporting.
Single-module Training Activity
For an activity containing one Course or Event module, configure Questionnaires from that module's Gear or Features menu.
Some features that conceptually describe the entire activity are placed on the module when there is only one module. If you cannot find Questionnaires at activity level, check the module.
Learning path: activity-level feedback
Use activity-level feedback when participants should evaluate the program as a whole after completing the complete journey.
Examples:
- Overall satisfaction with a certification program
- Whether a blended learning path achieved its objectives
- A final NPS question for the complete customer academy experience
- Overall coordination across multiple instructors and formats
The activity-level questionnaire relates to the whole learning path rather than one module.
Learning path: module-level feedback
Use module-level feedback when the feedback must be attributed to a particular Course or Event.
Examples:
- Evaluate each instructor-led workshop separately
- Compare virtual and classroom sessions in the same program
- Collect a pulse check after the first module so the next session can be adjusted
- Assess a newly redesigned Course without changing the program's overall survey
More than one module can have its own feedback configuration. With the default setting, participants can submit once for each configured feedback instance. A platform-level configuration can instead allow repeated submissions at activity and module level.
Combine module and activity levels carefully
A learning path can use both module-level and activity-level feedback. This provides detailed and overall insight, but also increases the number of requests shown to participants.
Use both only when the answers lead to distinct actions. Keep module questionnaires short and reserve the activity questionnaire for conclusions that require the complete journey.

Create the questionnaire first
Training Feedback selects an existing questionnaire. Create and validate the questionnaire before editing the Training Activity.
- Open Course Administration → Questionnaires.
- Select Create.
- Enter a clear internal and respondent-facing title.
- Add and arrange the questions.
- Translate the title, questions, help text, and answer options for the languages you support.
- Decide whether respondent names and email addresses should be visible in Analytics and reports.
- Configure anonymization and retention where required.
- Preview every supported language.
- Create or save the questionnaire.
The Questionnaire builder provides several question and presentation types. Common Training Feedback choices include:
- Rating Scale or Linear Scale for satisfaction, confidence, relevance, or instructor evaluation
- NPS for likelihood to recommend
- Single Choice, Multiple Choice, or Dropdown for structured categorization
- Short Answer or Long Answer for examples and suggestions
- Number for a defined numeric measure
- Heading, Text, and Divider for structure and guidance
Additional types such as Consent, Date, Address, or File Upload are available for specialized questionnaires, but usually require a stronger privacy and business justification than ordinary training evaluation.
Question-specific Required means the respondent cannot submit the questionnaire without answering that question. This is different from Sending feedback is required to receive the certificate, which controls whether the complete feedback submission is a prerequisite for certificate delivery.

Reuse or create a dedicated questionnaire
Reuse a standard questionnaire when you need comparable data across activities, instructors, regions, or time periods. Create a dedicated questionnaire when the learning design or decision requires unique questions.
A useful model is:
- A short global core: relevance, quality, confidence, and recommendation
- Format-specific questions: virtual delivery, classroom logistics, self-paced design, or instructor facilitation
- One optional open question: the most important improvement
Avoid creating one survey per scheduled delivery when the questions are identical. Reusing the same questionnaire allows Analytics to aggregate results while the activity context can still be used to segment reporting.
Use revisions intentionally
Questionnaires are versioned. Structural or logic changes create a new revision so historical answers remain valid.
After a revision:
- Earlier responses remain available in reports with their original revision.
- Overall response totals remain cumulative.
- Current question-level charts start from the new revision where the structures cannot be compared safely.
- Historical answers are not rewritten to fit new questions or answer options.
Make minor wording corrections carefully, and avoid changing scale direction or meaning in the middle of a live evaluation period. If a material redesign must remain analytically distinct, clone the questionnaire and give it a versioned title.
Connect Training Feedback
Connect at Course or Event module level
- Open Course Administration → Activities.
- Edit the Training Activity.
- Open the supported Course or Event module.
- Select its Gear or Features menu.
- Enable Questionnaires.
- Select the questionnaire.
- Configure Availability.
- Decide whether submission is required for the certificate.
- Decide whether instructors should provide feedback.
- Save the module and activity.

Connect at learning-path activity level
- Edit the multi-module Training Activity.
- Open activity-level Features.
- Enable Questionnaires.
- Select the questionnaire.
- Configure Availability, certificate dependency, and instructor feedback.
- Save the activity.
Preview is useful for checking placement and wording, but it is not a real submission test. Use a registered test participant to validate completion, certificates, email reminders, and reporting.
Understand every Training Feedback setting
Questionnaire

Dependencies and special cases
- The questionnaire must already exist and be visible within the administrator’s permissions and organization scope.
- Reusing a questionnaire combines responses in questionnaire-level Analytics. Filter by activity or usage context to isolate a particular delivery.
- Replacing the selected questionnaire changes the form for future responses. Review historical reporting before changing a live activity.
- Deleting or materially changing an in-use questionnaire can affect the participant flow, rules, and current analytics. Follow the platform’s validation messages and verify every connection.
Feedback availability
Availability controls when the participant can open the feedback form.

Important dependency
Because submission is allowed early, do not combine this availability setting with certificate gating unless you intentionally accept feedback submitted before the learning is complete.

Completion signals and dependencies
Completion can be recognized through applicable signals such as:
- The scheduled Course or Event reaching its end date and time.
- The participant fulfilling the certificate criteria.
- The participant completing all required content when no separate certification requirement defines completion.
For an instructor-led Event, configure attendance, schedule, completion, and certificate behavior consistently.
Require feedback before certificate delivery

Participant flow
- The participant satisfies the normal training or certification criteria.
- Feedback becomes available.
- The certificate remains unavailable until the participant submits the connected feedback.
- After submission, the certificate can be issued if every other criterion is satisfied.
Dependencies and special cases
- A certificate must be connected at the corresponding Course, Event, or learning-path level for the setting to have an outcome.
- Question-level Required settings determine which answers must be completed before the questionnaire can be submitted.
- By default, the feedback can be submitted once. The form and its translations must therefore be correct before participants reach it.
- A platform-level configuration can permit repeated activity- and module-level submissions.
- Instructor feedback is separate from the participant’s certificate condition unless another process has been deliberately configured.
- Force completion is an administrative exception: when an authorized administrator marks the training complete manually, required Training Feedback can be bypassed and the certificate issued. Use force completion only within an understood audit and governance process.
Use certificate gating proportionately
Making feedback mandatory can improve response rates, but it can also encourage rushed, low-quality answers. For regulated or externally accredited training, confirm that withholding evidence of earned competence for an evaluation is permitted by policy.
Instructor feedback

Dependencies and special cases
- Assign the correct responsible instructor at the same operational level.
- Instructor visibility in Questionnaire Analytics and reports is limited to activities for which the instructor is responsible.
- Write questions that make sense for the respondent. If participant and instructor questions differ substantially, use separate, clearly named feedback designs at the appropriate levels instead of presenting ambiguous wording to both audiences.
- Instructor feedback does not replace attendance marking, assignment assessment, practical evaluation, or participant feedback.
The participant experience
Before feedback is available
With only after completing training, the participant does not receive the active feedback action until the applicable completion condition has been reached.
If the participant expects feedback but cannot see it, the usual cause is an incomplete Course, unmarked Event attendance, an unmet certificate criterion, a schedule that has not ended, or a configuration at another level.
When feedback is available
The participant sees a Leave feedback action in the relevant training interface. An automated email can also contain the Activity leave feedback button token, which links the participant to the applicable feedback flow.
The questionnaire uses the participant's available language and the translations configured by the questionnaire creator. Missing translations can expose fallback content or make the experience inconsistent.
After submission
- The response is stored under the questionnaire and its activity or module usage context.
- By default, the participant cannot submit that connected Training Feedback instance again. When repeated submissions are enabled platform-wide, each additional response is stored separately in Analytics and the Questionnaires Report.
- A held certificate can be issued when all other criteria are satisfied.
- The participant no longer belongs to the Participants not sent feedback audience for that instance.
- Questionnaire rules and configured notifications can run where applicable.
- The response becomes available in permitted Analytics and reports, subject to privacy and anonymization settings.
Participant View and administrative Preview
The administrator's participant preview can show the Feedback icon, but Send and Cancel are disabled. This prevents preview activity from creating a real participant submission.
Use Preview to inspect placement and appearance. Use a properly enrolled test participant when validating submission, certificate issuance, reminders, notifications, Analytics, and reports.
Certificate and completion interactions
Training completion, feedback availability, and certificate issuance are related but separate states.

If a certificate is missing, determine which state is incomplete instead of assuming the questionnaire failed. A participant whose signup has become Expired can still submit outstanding required Training Feedback from the profile and receive the certificate when all other criteria are met.
For a learning path, check whether the certificate and feedback were configured at activity level or on a particular module. A module-level feedback submission does not automatically satisfy an activity-level feedback dependency, and the reverse is also true.
Remind participants who have not submitted feedback
Use an Automated Email Workflow to prompt the correct audience at the correct time.
- Open Course Administration → Automated Email Workflows.
- Create or edit a workflow.
- Add a rule with Who = Participants not sent feedback.
- Choose a viable trigger relative to Start date, End date, Deadline, enrollment, or participant completion.
- Write the subject and message.
- Insert the Activity leave feedback button token so the recipient has a direct action.
- Save the workflow.
- Attach it through Automated email workflow at the same activity or module level as the feedback process.
- Test with one participant who has submitted and one who has not.
You can add more than one reminder, such as:
- At participant completion
- Two days after the Event ends
- Seven days after the activity ends
Stop before reminders become excessive. A short questionnaire and one well-timed reminder often produce better data than repeated messaging.
Match the rule to the configured level
Use activity-level schedule and tokens for final learning-path feedback. Use module-level dates, instructor, location, and title tokens for feedback attached to a specific Course or Event.
If the rule depends on a date or status that does not exist at that level, it is skipped. Review the activity's notification timeline for the reason.
Workflow edits apply to existing enrollments for future triggers. Triggers already well in the past are not replayed retroactively; a short grace window can apply to recently passed triggers. Use a manual message when an older cohort must receive a newly created reminder.

Notify administrators about submissions
Platform-level Questionnaire settings can notify administrators when Training Feedback is submitted.
Send Email About Training Feedback
When enabled, every Training Feedback submission can trigger an administrator email. The setting applies to questionnaires used as Training Feedback, not to every questionnaire on the platform.
Use it when individual responses require prompt review. In a high-volume environment, sending an email for every response can overwhelm the recipient and hide the cases that matter.
Send Email on Low Ratings Only
Where available, this sub-setting limits notifications to submissions containing a Rating Scale or Linear Scale answer at or below a configured threshold.
The threshold can be expressed as:
- An integer, such as 3 or lower
- A percentage, such as 30% or lower
Use low-score alerts for service recovery, instructor-quality exceptions, customer-facing certification programs, or operational incidents. Define who owns the response and how quickly they should act.
Email templates and recipients are configured in Settings → Email Sending → Questionnaires. A shared monitored inbox is often safer than one person's mailbox.
A low score is a signal, not a conclusion
Review the open comments, response context, sample size, activity, instructor, region, and trend before taking action based on one value.
Privacy, identification, and anonymization
Training Feedback can contain personal data even when the questionnaire does not explicitly ask for a name. Activity, instructor, organization, language, timestamps, free text, and small cohort size can make a respondent identifiable.
Plan privacy at questionnaire creation rather than after responses arrive.
Show respondents' names and emails in Analytics and report
Enable this questionnaire setting when authorized reviewers need respondent-level follow-up.
When enabled:
- Permitted Analytics users can use See details for supported question types.
- Reports can include respondent names, email addresses, identifiers, organization, and contextual fields.
When disabled:
- Analytics remains aggregated.
- See details is hidden where respondent identity would be exposed.
- Names and email addresses are restricted in the report according to the questionnaire's configuration.
This visibility setting is not the same as deleting data after a retention period.
Schedule anonymization
Anonymization controls what happens to stored data later.
Choose between:
- Anonymize user data only: removes profile-linked identifiers while retaining the answers.
- Anonymize user data and responses: removes the response data as well, including uploaded files.
Timing options include:
- Immediately after submission for user-data anonymization
- A defined number of days after submission
- A specific date and time
Retaining answers without profile identifiers supports aggregate quality analysis. Full removal supports stricter retention policies when the responses no longer have a lawful or operational purpose.
Anonymizing profile links does not remove personal information that a respondent typed into an open answer or included in an uploaded file. Avoid asking for unnecessary personal data and explain what respondents should not include.
Activity-connected feedback versus anonymous URL
Training Feedback should normally be attached through the activity or module feature. This preserves the training context and supports availability, controlled submission behavior, certificate dependency, reminder audiences, and reporting by activity.
A copied questionnaire URL is useful for external or anonymous audiences, but it is a separate distribution route. A manually shared link does not provide the same technical relationship to the Training Activity. Do not use it as a substitute when certificate gating or activity-specific completion tracking matters.

Analyze and report on feedback
Use Analytics → Questionnaires for visual monitoring and the Questionnaires Report for exact, exportable records.
Questionnaire Analytics
Analytics helps answer:
- How many responses have been submitted?
- What is the response rate?
- Where is the questionnaire used?
- How are answers distributed?
- Are results changing between activities, organizations, or periods?
Questionnaire-level Analytics can show:
- Number of questions
- Number of responses
- Used-in count
- Response rate where an audience can be determined
- Per-question answer and response rates
- Charts for supported choice, scale, rating, and consent questions
- Respondent details when privacy settings permit them
Only questionnaires with at least one recorded response appear. The same questionnaire can aggregate multiple usage contexts, so filter to Activity and the intended activity or module when evaluating Training Feedback.
Questionnaires Report
Use the report for respondent-level review, audits, export, external analysis, or qualitative coding.
Relevant filters include:
- Submission date range
- Questionnaire
- Usage type, such as Activity
- Specific activity, Course, or Event
- User type: Learner or Instructor
- Anonymous responses where supported
- Report template and columns
The report is row-based: each row represents one question response, with contextual fields such as questionnaire revision, entity, respondent, organization, timestamp, question, answer, score, and language where permitted.
If multiple questionnaires are selected, each can be exported to a separate worksheet.
Role and organization access
Analytics and report access follows permissions and organization scope.
- Administrators see only the questionnaires, activities, and organizations they are authorized to access.
- Parent organizations can see permitted suborganization data; lower-level organizations cannot see upward or sideways.
- Instructor access is restricted to feedback connected to activities for which the instructor is responsible.
- Respondent privacy settings apply consistently across roles.
- Blocked-user responses remain for historical accuracy; deleted-user responses are excluded. Cancelled and expired signups can remain represented.
NPS
The NPS question type supports transactional feedback about one activity as well as broader relationship surveys.
For Training Feedback:
- Add an NPS question to the questionnaire.
- Attach the questionnaire to the relevant Course, Event, or activity.
- Collect responses through the normal feedback flow.
- Use Analytics → NPS for the score, promoter/passive/detractor distribution, trends, and organization breakdown.
- Filter by activity, questionnaire, training type, organization, registration status, or period for meaningful comparison.
Responses with personal details hidden still contribute to aggregate NPS totals but are excluded from user-level filters and organization breakdowns where identity is unavailable.

Design feedback that leads to action

Use representative accounts and the routes enabled for your delivery. Check the relevant outcomes before launch and after a material change; an administrator preview alone does not verify the participant experience.

Where do I configure Training Feedback for a single-module activity?
Open the Course or Event module and use its Gear or Features menu. Questionnaires may not appear at activity level when the Training Activity contains only one module.

Should I configure feedback at module or activity level?
Use module level to evaluate a specific Course or Event. Use activity level to evaluate a complete multi-module learning path. You can use both when they answer different questions and the additional participant effort is justified.

Can I attach Training Feedback to an Assignment module?
The Training Feedback feature is supported for Course and Event modules and at the appropriate activity level. Use an Assignment for assessed work, or place feedback at activity level or in a supported Course/Event context.

Can I use the same questionnaire for several activities?
Yes. Reuse supports consistent comparison. Questionnaire Analytics aggregates the responses, while usage and activity filters let you isolate a particular delivery.

Can participants submit Training Feedback more than once?
By default, no.
A connected Leave feedback instance accepts one response. A platform-level configuration, Enable sending feedback multiple times, can allow repeated responses for questionnaires attached at activity or module level. The setting is off by default, but can be enabled by request to Eurekos support. It stores each response separately in Analytics and the Questionnaires Report. Enable it only when repeated pulse checks are intentional, and test reminders and reporting before launch. -
What is the difference between Feedback anytime and Feedback after completion?
Anytime makes Leave feedback available from enrollment and access onward. After completion waits until the applicable Course, Event, or activity completion signal is reached.

What makes feedback available after completion?
It depends on the training structure. Applicable signals can include a scheduled end, fulfilled certificate criteria, or completion of all required content. Check schedule, attendance, completion, and certificate configuration at the same level as the feedback.

Can Training Feedback be mandatory?
The questionnaire can be required for certificate delivery by enabling Sending feedback is required to receive the certificate. Individual questions can also be marked Required in the questionnaire.

Does required feedback prevent the training from becoming complete?
Its primary documented effect is to hold the certificate until feedback is submitted. Treat training completion and certificate issuance as separate states and verify both in the participant flow.

What happens after required feedback is submitted?
If all other certification criteria have been met, the held certificate can be issued. The submission is then stored for Analytics and reporting according to privacy settings.

Can an administrator bypass required feedback?
Yes. Force completion can bypass required Training Feedback and issue the certificate. This is an administrative exception and should be governed and audited.

Does instructor feedback control the participant's certificate?
No. Enabling instructor feedback creates an instructor response flow. It does not make the instructor's submission the participant's certificate condition.

Can instructors see all feedback on the platform?
No. Instructor access to questionnaire results is limited to activities for which the instructor is responsible, and remains subject to analytics permissions, organization scope, and respondent privacy.

Can I remind only participants who have not responded?
Yes. In an Automated Email Workflow, select Participants not sent feedback and include the Activity leave feedback button token.

Can administrators receive an email for every response?
Yes, if Send Email About Training Feedback is enabled in the platform's Questionnaire settings and the relevant email recipient and template are configured.

Can administrators receive only low-score alerts?
Where the sub-setting is available, enable Send Email on Low Ratings Only and define an integer or percentage threshold for Rating Scale and Linear Scale answers.

Is hiding names the same as deleting personal data?
No. Hiding respondent names and emails restricts visibility in Analytics and reports. Scheduled anonymization later removes profile identifiers or deletes identifiers and responses according to the selected retention rule.

Can free-text feedback potentially identify an anonymous participant?
Yes. A respondent can type identifying details, and activity context or a small cohort can make someone recognizable. Design questions and access rules accordingly.

Can external users complete the same questionnaire?
Questionnaires can be shared through a copied URL, including anonymous access where configured. That is a separate usage context and does not provide the same activity-linked availability, certificate, and reminder behavior as Training Feedback.

Why did current question charts restart after I edited the questionnaire?
A structural or logic edit created a new questionnaire revision. Historical answers remain in reports, but current question-level charts use the new structure so incompatible questions are not mixed.

Does Preview submit test feedback?
No. Participant Preview can display the Feedback icon, but Send and Cancel are disabled. Test with a real enrolled test user when validating the workflow.

Is NPS the same as the activity Rating feature?
No. NPS is a 1–10 questionnaire question with promoter, passive, and detractor analysis. Activity Rating is a separate 1–5 star feature that can display an average on the Activity Description and Storefront.

Can a participant with an expired signup still submit required Training Feedback?
Yes. If the participant met the certificate criteria but did not submit Training Feedback required for the certificate before the signup became Expired, the participant can still leave the outstanding feedback from the profile. After submission, the certificate can be received when all other certificate criteria are satisfied.
